How to extend Open Work Permit : Live-in Caregiver

Hi guys, I hope someone here can give some advice about the renewal of OWP. My friend is live-in caregiver and she already applied for PR on Feb2012. Her Open work permit received on May 2012 but will be expired on June 2013 same with her passport. She got a new passport (renewed) and she would like to renew her OPW. What application form she need to fill-up to extend her OPW? Thanks

It's the same form for the OWP. She just needs to make sure to write its an extension because of a new passport and include the new passport pictures.

No need LMO and contract if u going to renew ur OPen work permit... Just passport and application for open work permit

thank you HoneyGirl ...so ill just need the IMM 5710,,document checklist, passport photocopies,stamp receipt, and copy of work permit...

also how will i fill up th 5710 .. by that time i am still working as live in caregiver with my employer so i will just still put her as my employer ? ..

bec when ill apply my pr/owp next week i am still be using my current passport which will expire may 2014 so im assuming the expiration of my owp will be may 2014.. i am going home for vacation this november and i will renew my passport in phil to avoid hassle going to vancouver...so when i come back from ph i will immediately apply for extension of my owp..thats why i am inquiring now thank you for replying God bless you
