There this problem in Quebec is that they don't understand middle names in the same way as the rest of the country. If you declare a name as a middle name in Quebec, it won't show up in any document other than a birth certificate. If you want it to show up, you must declare it as part of your first name, and then it will show up absolutely everywhere. There is no middle ground.
actually just today I called my 3 friends (same birth country) becuase they got their citizenship few months back. and i found out that in mot alone, they have the same 'wrong' given name. all of their given name includes they middle names ha ha ha. Well i think its not a big issue at all :p :p :p

As others have said, the problem isn't the existence of the middle name (I also have a middle name which is listed on my passport and cit certificate).

The problem seems to be that you've lost your middle name and changed your surname to your middle name, though your RAMQ card still shows Velez as a surname.

If you're happy with Velez as a middle name and not worried about the contradictory documentation, you could probably go back another day and speak to someone else without incident.

Or else you could consider putting:
Given names: Dianne Char
Surname: Velez Diaz

This would clarify things when dealing with Québec and avoid confusion in comparison to other documents where Velez is a surname.

But it's ultimately up to you and what you want your name to be.

yeah i agree thanks. well ill just wait for my passport and if the passport shows DIANNE DIAZ then I will keep it as it is

can some one clarify me below

if a persons name is x y z
x = first name
y = middle name
z = last name

the certificate only has given name(s) and surname

so will the certificate show below

surname(s) = z
Given name = x

so do they just kill y and not mention it in the Citizenship certificate.

or is it like this below
surname(s) = z
Given name = x y

but passporting said it is wrong Y shouldn't be there

Mine is

surname(s) = z
Given name = x y

and no one ever complained. All my documents show either that or

x = first name
y = middle name
z = last name

The issue is that in hoping_canadian's case, y is not a given name, so it's not really a fair comparison.

I have 2 given names:

Given Names: A B
Middle Name: C
Surname: D

my certificate shows: A B D

that is what the passporting staff want to see the in certificate name and surname only.

i just found out the most of my friends is having the same issue. they didint do anything and they think it is ok. So i dont bother to send my certificate to correct it :D

Hello just want to ask how long it will take to correct your certification? In my case Im applying for a canadian passport for my son but the staff told me that they cannot process it, because when ever they scanned the certification everything is valid except the picture it appear my daughter picture instead of my son pic....myquestion isdo i need to submit new picture for my son or just only the picture attached on his certification? Any help please and thank you!

Can you fill in just the given name instead in the passport application? Or do we have to include the middle name under the given name? Our certificate have given name-middle-last.

in the application as far as i remember they will ask you only the given and sur names
