Help with medical inadmissibility-Declaration of ability and willingness

Hi Guys,

A couple of months ago I got a medical inadmissibility procedural fairness letter for my PR application, and I'm preparing the response to be submitted soon,..

I want to fill the "Declaration of ability and willingness form" but there is a field which says:
----------------------
Commissioner for Oaths
(with seal)

Does anyone know where can I get that?
