Help needed. Submitting ITA, need help with a couple of questions.

1. Regarding the Travel History section, should I include all the cities I visited during a trip to a particular country, or is it sufficient to list the entry city? For instance, I recently traveled to Turkey and landed in Istanbul, but during the same trip, I explored several other cities by plane. Should I list all the cities I visited by plane during the same trip, or is Istanbul alone adequate?
2. In the Statutory Questions section, there is a query that asks, "Have you had any serious disease or physical or mental disorder?" I have epilepsy, and I disclosed this information to the physician during my medical examination. Should I respond with "Yes" to this question, even though I haven't experienced a seizure in over six years?

I used to study in the US, and travelled to another US city that’s not where my school is located. In my PR travel history, I only listed my school’s city since that’s where I spent 95% of the time. My PR is approved, so I think it wasn’t a problem
