Yes may forms, Appendix A and B. I only sent my new passport. Isusulat mo naman Sa appendix yun. Sakin nga I also changed my number. No problem with that, as long as you clearly state it on your appendix A and B.

here's my timeline when CEM requested for additional docs for my spouse.

June 18, 2015 CEM requested for additional documents (spouse questionnaire, spouse's PCC)
July 10, 2015 Additional documents sent to CEM
July 13, 2015 CEM received the additional documents

Nasa letter ng MR ang description. visa canada passport size. pareho lang nung pinadala natin sa application.

I suggest Canadian nalang since you'll be submitting the same specs for your PPR.

Good morning po sa lahat. Can anyone help me out regarding my medical results? It says in the letter that we have to undergo medical examination within 30 days. What if I have follow-up medical examinations because SLEC Ermita needed medical certificate from my private Ob-gyne. I have done that last August 18, 2015. And SLEC Ermita told me that it won't affect my application since I have undergone medical examination within that 30 days. SLEC did submitted the medical result of my husband and 2 kids. Any opinion po? Would appreciate very much. God bless us all. And congrats to all the good news.

Hi.. Just want to share my experience..
I and my son also have a follow up next month at SLEC BGC. This is for the vaccines (MMR) which is a requirement upon landing. But our med results were already send by SLEC last 14-Aug-15.

Just ask SLEC Ermita if they already transmitted your results to the embassy and when did they send.
