For other mebers who asked what it stands on transaction, here what is says on mine:

Description: IMMIGRATION CANADA FSW, course: MC 28.06.2013. 1 USD = 1.050100 CAD, 1 USD = 0.766460 EUR
Original amount: 1.100 CAD (yup, it is in CAD, not in USD as some people thought)
Amount to pay: 802,88 €

As per my understanding, Education evaluation is required only for the highest degree according to the NOC. For example if you are showing the ECA report for graduation, then you can skip the report for higher secondary education.

Yes it is mandatory. You have to attach a bank reference letter along with evidence of readily transferable funds in the form of bank balance statements with final balance exceeding the amount specified by CIC.

Per peson reqd amount means applicant 550$ + spouse 550$ and 150$per child
You need to show as bank balance or fd or any liquid fund
