Hi, i woul like to know which forms I need to submit for an open work permit for my spouse??? I think is the format IMM 1295 ??? is it ok? I need other else? IMM 5645 too??? or just for me...plz help me.

For a open work permit for your spouse fill out the IMM 1295E form. There's a option in the form for open work permit.
About the IMM 5645. Get a Visa Application Kit from the Embassy of Canada in ypur country.
If the Application Checklist mentions the IMM 5645 form it means that you and any person over 18 (your spouse) years old must fill it out.
