Format for Degree / PG transcripts

Do we have any format for Degree / PG Transcripts which we have to get form University.
Please help

Do mean photocopy of marksheet, degree & course completion certificate?

Whether it has to be attested by the University?

1) All marks sheets of your degree semester wise
2) Provisional Certificate
3) Consolidated Marks list
4) University final Memo degree

transcripts will be given by university ... every university has their own format and send it should be in sealed cover and stamped by university .
