So when I submitted my first IRCC application (study permit), I didn't put my late step-sister details in the siblings section because I thought she was late and there was no need and also due to my inexperience and ignorance.
Fast forward, now am applying for permanent residency and I want to add her because IRCC specify clearly on form IMM 5406e (Additional Family Forms) that " (if deceased: give city/town, country or territory and date) ". If I rectify this, will IRCC see it as misrepresentation or I should just ignore it as I did earlier.

As a general rule of thumb an honest mistake must be identified to IRCC as soon as you realise it. I don’t think your sister’s details would have made any impact on your SP.
You can safely explain it in your PR application LOE as you explained it here and provide details in the form. The other option would be to continue to hide these details which I’d never do.
