Do I need Medical Examination?

I have been in Canada since last 2 years plus and only traveled back to India for 2 months about 11 months back.
I now have an invitation to apply for permanent Residency, and they have asked for a medical exam.

is it mandatory to get an medical exam done if you are in canada itself?

But the forms to be uploaded says medical certificate as well. How can we confirm
