The instructions is very clear. You will have to sponsor your family member abroad after getting your permanent resident . Only family members in canada included in your application will get permanent resident on the same day as you.

Its very normal. They are been examined.
All of your family member must have a medical exam done regardless of whether or not they’re coming with you to Canada.

All your dependants must have a medical exam done to make sure they’re admissible to Canada. They must be admissible to be able to be sponsored in the future.

Can one sponsor a dependant in Canada that wasn't included in application because he is over the age of 22?

Yes you can sponsor a dependant who is 22 years of age or older if they have depended on their parents for financial support since before the age of 22, and they are unable to financially support themselves because of a mental or physical condition.
