CEC - IMM 5475 Visa office location? help?

Hello,

I got a request to provide IMM 5475 form, and I'm wondering about the name of the office where I submitted my application as well as the location of the office.

I submitted my CEC application via MyCIC (while residing in South Korea), and from what I can tell, everything goes to Manila. So does that mean the name and the location of the office is Manila? Should I write down that for both?

Oh, they do the eligibility requirement stage - good to know.
It seems like it's not a big deal if I make a mistake in terms of the location and the name of the office, but I'll definitely put Sydney.

Similar situation.
What should I fill for the following fields:

Name of office where the application was submitted
and
Location of office

I understand from your previous reply that Location is Sydney, Nova Scotia, Canada... but what's the name of the office??
