Bearing original signatures???

Hello guys,

I filled out an IMM 5475 form, and just wrote my name down in the signature section, and now they are requesting me to do it again. I assume I have to print it then sign it with a pen then scan it?

yes, I have requested the same... please advise...IMM 5669e is a barcode form still suppose to validate and print it sign it and scene and upload? please advice
