Background/Declaration form (IMM 5669)

Hello,

I am looking at the Background/Declaration form (IMM 5669) and I have one question to ask...
In this form, it says "Provide your detailed personal history from the past 10 years (jobs, study, vacation and etc)"

I know I need to fill out this form, but does that mean I should also attach original letters from previous employers and transcripts from my high school to prove?

Any suggestions? Please let me know and Thanks!

You only need letters from your employers in the past 3 years before your application. Transcripts and school documents are not required.

The information requirements of the forms are distinct from the supplementary documents required. Refer to the document checklist for required documents.
