Hi Matet, first u have to fill up the form for re-entry visa, you need a letter from ur employer as proof that u have a job in canada which is very important, u have to call the call center of the canadian embassy in manila to schedule the pick up of ur documents, the form for re entry visa, letter from employer, copy of work permit, passport, pictures, copy of ur flight schedule, and the payment as well i think it's 75$ converted to peso depends on the forex rate, hope this helps, goodluck.
