Apply family visit visa for my mother

I am applying the family visit visa for my mother.
I want to act on behalf of her because she doesn't read English. I used my sign-partner account to set up her application.
In the bottom of IMM5257 should I sign her name or my name?

You should create an entirely new account for your mother and appoint yourself as representative.
