Application form of renewal of PR card

I am looking for an up to date application form for renewal of PR card. Can someone send me an attachment please, I am trying to open it on CIC website so far i am unable to open it.
how long in advance should i apply before it expired. email meganuvin at gmail dot com

In case the pr card get expired i can still renew it any suggestion please
