I would just like to ask, if CEM asked for Appendix A is it a sign that my application is already approved and that a visa comes after?

The appendix A is in a table form where you need to fill up family and given name, dob, pp details, eye and heigh :

The appendix A is in a table form where you need to fill up family and given name, dob, pp details, eye and heigh :

Yes. Everything seems fine. The Appendix A usually comes with Passport request. If they send you this Appendix A along with Passport sending instruction, then most likely you are approved and they will stamp visa in the Passport.
