You see why I think the mix up might be from DHL,if you paid for them to bring in your package,you give them a delivery address and that's what they are supposed to use,not the address on your application.if they adhere strictly to the address provided by the customer,irrespective of the address...

It's the generic form.but it's not compulsory you send if family composition hasn't changed. In my case,I sent even though family composition hadn't changed but someone else I know didn't send yet he got his visas.

Its the address you give when authorizing DHL for pickup that will determine where your passport will be sent to and not necessarily what AVO writes.
