Seems IRCC has slowed down. Am bit surprised with your timeline where biometric and medical dates are after pre-arrival. Did they ask for a new police cert ?
Well.....I can't say its asking for a new police cert as that was the only one I sent but the message in the tracker stated as a replacement is required though......really don't know which part went wrong with IRCC

how long after receiving AOR we can create an account and link the application? Should sponsor created the account or the principal applicant? Where should we create an account for tracking the process?

You may use this tracker now https://prt-srp.apps.cic.gc.ca/en both applicant and sponsor has to register his/her own account.
Thank you for the link. I tried it with both applicant and sponsor information but could not create account for either one. I keep getting error to correct information. I am sure that I have entered the information correctly. I will try again tomorrow.

Do both applicant and sponsor receive AOR emails or just sponsor?

I tried the IRCC secure login for both PA and sponsor. I could create the accounts fine but when I searchfor application, it can not find the application to link.
my guess is that, may be we should wait till PA also receives AOR. Now only sponsor has received AOR.

2. It says to submit the documents at VFS near us. VFS in Mumbai only allows Appointments for Biometrics. How can we submit this package?
