Hello, i got my medical tests done here in US yesterday. I noticed today that in my information sheet, they've entered my birth date incorrectly :( . Ideally i should've checked it properly the moment the lab assistant shared that document with me. I'm afraid of the repercussions here. I'll call them up tomorrow and see if this mistake can be corrected. Would like to know if anyone has faced this situation before? How did it get resolved? Even if no one has faced this situation before, do you think this can be fixed and if so, how?

It's nothing. Attach a letter of explanation when you submit your medical at the very least, or alternatively you can have the clinic correct it and print you out a new sheet. Again. It's inconsequential.
 
Thanks! They corrected my birth date however later made a new gaffe and changed my passport country. When i told them to change this, they said they are having system issues. It's just carelessness on their part ,nothing else.
