wrong date on appendix A

Yup, i just sent our application. Upon double checking, yes Appendix A should be included. I missed the ID pics too

we sent each and every document but without appendix A, as all the documents mentioned in appendix are also mentioned in the check list IMM5491E

It is mentioned on check list IMM5491E that it should be sent with the application but not on the appendix A.
