Workpermit from outside Canada, waiting for the medical test form from Consulate

Hi,

I am applying for a Work Permit to canada from India. I am ready with all the documents but I am waiting to take medical tests for which i need a form with a file number. I have sent an email to consulate for the medical form but its been 5 days that i dint receive the form.
can anyone suggest any alternate method to obtain this form. Or Can I know to what email id do I send the email?

Hi,

It seems from your post that you are yet to file your papers at CHC. The medicals are done once you have filed your work permit application. The form & the file number will be generated after you file your papers with CHC along with processing fees.
