My former employer is driving me crazy. I have every other document ready including upfront medical exam. Then, this guy is taking more than 1 month to give a stupid work reference letter! Whenever I ask him, he says he would call me and when I try to call he wouldn't answer. This guy'd never given me ROE and lost all of my paystubs. Now, I only asked for one letter and he is taking forever!!!!! urg!!!!!!!!!!!!!!!! so frustrating!!!!!!!

You can ask a former colleague if not your former boss to give you a reference letter. If the reference letter by your former colleague is not on a letterhead, you can get it notarised and upload it. For proof of payments, you can include your copy of paystubs.
