Work permit Error

I am sponsoring my spouse and filling work permit application. In one part its written to mention " date and place of your original entry" which I mentioned and in same section date and place of your recent entry needs to be mention.

My original entry and recent entry is same and but if I mention same date it shows error. Can someone please help me out?

I don't have the form in front of me now, but doesn't that question say something like, "Date and place of most recent entry, if different from original entry?" That's at least how I remember it. If it's still the same, then you should leave the most recent entry blank, as it's the same as the original one.
