Can we have to declare in application which we work on off campus wp during study? Can we need reference letter also?

Only work experience on PGWP can be counted towards CEC experience.
Please refer to this topic: http://www.canadavisa.com/canada-immigration-discussion-board/work-done-prior-to-pgwp-t162215.0.html

That's I know but in application form we have to write last 10 year history so if study and work on off campus wp so what I write study or work or study and work?

Correct. Schedule A - Background Declaration you will have to provide information for the last
10 years. I would put study in and 'work (off-campus wp) or something.
I'm not sure whether I would mention that position on off-campus WP in Schedule 8 form.
Maybe someone else can helps us here?

Schedule A, Personal History - you can list work and study as separate items. It's OK to have overlapping dates, just no gaps.

If the work was in a SKILLED NOC and was in the past 3 years, yes, you put it on Schedule 8.

if job is not skill than can we have to dclare?

Yes, like jes_ON said, ALL jobs, skilled or not, have to be put into Schedule A: Personal History.
