With PR card plannin to go out of country for a month

hi guys,!

I am a FSW under medical surveillance! I've had my first appointment and everything went well. I am rescheduled next year 2013 for another tests. To get to the point, I wanted to go out of the country for a month, I have a valid PR card and i was wondering if i need to inform CDC if i will be going out of the country?
