Which name to use? Previous married name or ?

Let's call her

"Ann Sacapano Mindoro"
Her middle name above is her Mother's maiden name.

After she got married, her name changed legally which is stated in her passport.

"Ann Mindoro Nepalese"

Her last name changes to her middle name and she assumes the new married last name

What name would she use on all the applications?

For all things such as our joint bank account, her postal ID, police clearance etc. She uses her non-married name. Only her passport states the married last name.
Anyone see any problems with this?

I believe the name you use on the application needs to match the name on her passport. However hopefully someone else can confirm and comment on the rest.

yes ur name in application should be the same as in ur passport

For the application you use the name on her passport, but there is a place on the forms that asks you for other names you go by, so she can state there the name she uses on her other documents. You can attach a note explaining the situation as well.
