Where should I send my PCC and the other supporting documents(planning to send latest T4 etc) for PR application?

Do you have the link where it is mentioned that I should send PCC here.

How about the other documents? Should they be sent separately or at the same address.

Also in same envelop or a separate envelop for PCC and other docs.
