sushsmart15 has mentioned, there is no specific bank certificate format. But I would suggest that you get a format drafted and take to the bank so that you get what you want on their letterhead, they may tweak it a bit, but as long as you have the required information, it should not matter.
You can modify the below format to suit your requirement. You can get the following on bank letter head.
