I just submitted my EE profile last week (CRS 457) and will be taking TEF in 11 days. I also just submitted the request for Police Certificate to the FBI, which will take months for me to receive.

Should I try to schedule the medical exam now or wait until I get ITA?

Also, what is involved in the medical exam?

My medical centre wouldn't do the medical exam without the letter of invitation, then again every medical centre is different. Up to you, just be careful with its validity.

They do: physical, blood test, urine test and X-rays.

Cheers and +1 me bro ;)

You should wait for your ITA. Refer to this link from CIC website http://www.cic.gc.ca/english/information/medical/medexams-perm.asp

I saw this but it says you can also do it "up front", that's why it's a bit confusing!

The best time to do the medical exam is once you get your ITA.

This should hopefully be clear enough for you:

"Do not complete any medical exam until you get an Invitation to Apply.

[...]

Note: The results of your exam are only valid for 12 months. When you submit your application for permanent residence, the results of your exam must be valid for at least 6 months. If we approve your application, but you do not come to Canada as a permanent resident during those 12 months, you may need to have another exam. For this reason, it is a good idea to complete the exam as close as possible to the date you submit your online application."
