what to write in a letter of authorization to collect passport by representativ

I want to collect my passport by another person from vfs. But a letter of authorization is required for that. But what should I write in that?????? please help!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

You should write that

Dear Sir/ Madam

I cananot collect my passport due to some domestic work/problem . Mr. XYZ identification# /ID# will collect passport on my behalf.
