Hi folks,

Sorry to bug you all with this,but I just want to double check.

I have my medical coming up,and the lady I booked it with on the phone just told me to bring my passport,2 passport photos and "any documents immigration have given me"...

I want to double check because she was one of those people who kept changing the story and telling me different things...is this all I need to bring?

Also,I am confused as to bringing any documents..I can't see any listed under my checklist?
We are applying spring sponsership of common law through the London office.

Any help is greatly appreciated guys!i don't want to get all the way out there and b told I don't have the right documents!
 
I am just confused as to weather I have to bring a form called "appendix C" ,I've read I have to take it,but I can't find it anywhere,plus they didn't tell me to bring it?
 
I had my medicals done a week ago and I brought Appendix C that I was told to bring with me. The doctor looked at the paper, laughed and wondered where I got this form since it hasn't been in use since 14 months ago. If you're doing your upfront medicals you will fill in the forms together with the doctor at the office.

The appendix C said to bring old medical records and medical subscriptions, but the doctor didn't ask me any. I guess it's more if you have had something serious or are taking medicine for something.

If you wear glasses, you should bring them with you since they check your eyesight (with glasses)!

At least for me, that was it. I had to bring all together 4 pictures, but my doctor used the old fashioned paper-forms, so guess he needed some more.
 
Just had our medicals done for upfront spousal outland,but had then done in Ontario. Took passport and the amount the Dr wanted for payment in cash .She did all the paper work and took photos .Applying through London office.
 
Cool!thanks for the info guys!went and did it the other day and all went great.

And thanks for the tip to bring my glasses!would have thought of that otherwise!

Good luck. Everything will be fine. I went to my wife's meds and it was pretty smooth. You can call ahead and ask them what they require if you have any doubts. The medical centers are pretty experienced in the documents they require to complete the exam.
