What makes a country "unable to provide the protection"?

In the Claimant's Guide one of the conditions to be able to apply for refugee status is to be "unable .... to get the protection of your home country." I remember that I read somewhere that the Canadian authorities decide that certain countries have just local problems (and thus people from there are likely not eligible to become a refugee in Canada, they are just supposed to move within the country) while certain other countries are assumed to be the disaster zone completely and thus unable to provide the protection.
I cannot find the source for this now. Please help me to clarify which branch of the government decides that certain countries are completely unsafe and what this status is officially called?

IRB knows already knows in what country what happens and judge knows as well.
