What is the Procedure for Applying Canada PR from USA

Hi, Could someone please let me know the procedure for applying Canada PR from USA.
I currently have 10+ years in IT(Information Technology) and living in USA on H1 B Visa and My wife has done MBBS in India and she is now preparing for USMLE, Step 1 and she is on H4.

Also appreciate, if anyone advise me if there is way to apply Candaian PR for doctor, who has completed Bachelor of Medicine and a Bachelor of Surgery(MBBS) from INDIA.

However, as of now of now CIC is not accepting any applications under FSW.
