When filling the “Family Information” form, how does it work in cases of the mother dead? I mean, in this section it is required:

Name
Dt. Of birth
Place of birth
Marital Status
Present Address

What am I supposed to write in the “Present address” field in case of a dead person?
Also, the first column of the section is the “Name” field. Am I supposed to write name and surname of all family members?? Or just the name?....some of family members have different surname from mine.

Please help.
