What if I use original extra sheet for sibblings? Urgent

Pls let me know in this case where I use original extra sheet what I did simply filled all columns with N/A
and additional two brother data in respective colomn. Is that ok.

My opinion - that might confuse the IO when they see one form with parents name and one form with no parents name.

Rather add an additional sheet of paper, type your name, form name, form number of top.

Write question number, recreate the table but only for your two other brothers.

When you write question number, write continued as well.
