What forms to fill out?

We (Me and my common-law) have an invitation from my sister to visit Canada.

My sister sent me some documents to fill out:
IMM5484E.pdf
IMM5721E.pdf
IMM5257E.pdf
IMM5645E.pdf
IMM5257B_1.pdf
IMM5409E.pdf

and upon checking the cic site h t t p ://w w w.cic.gc.ca/ english /information/ applications/ visa.asp
it has some additional documents on top of what my sister sent me:

IMM5484E.pdf
IMM5721E.pdf
IMM5257E.pdf
IMM5645E.pdf
IMM5257B_1.pdf
IMM5409E.pdf
Photo Specs
IMM5476E.pdf
Applying for Visitor Visa (Temporary Resident Visa - IMM 5256)


So I went ahead and had an Eligibility Assessment and this are the documents/forms required for me
IMM5257E.pdf
IMM5645E.PDF
IMM5409E.pdf
Digital Photo
Itinerary, Travel
Purpose of Travel - Other
Proof of Income
Proof of Means of Financial Support
Passport or travel document
Travel History
Employment Reference Letter
IMM5713E.PDF

and for my Common Law:
IMM5257E.pdf
IMM5645E.PDF
Digital Photo
Passport or travel document


My question is, which one to follow?

And while browsing the documents in the site (2nd set of documents), there is this form (IMM5257B_1.pdf) and while in the forms required by the assessment, there is none. Should I still submit IMM5257B_1.pdf or should I only submit forms given by the eligibility assessment?

Btw, I am from the Philippines.
