You must answer 'Yes' to both questions and give the details/reference the refusal letter in the space provided in the application form.

If you were refused visas/denied entry to any other country (not just Canada), you must declare those refusals as well

B) You have been refused visa earlier
C) Yes, you have previously applied for visitor visa and it was denied.
