Visitor visa extension

Hello friends

please guide me the process of visitor visa extension
I am from Calgary AB going to apply for my mother in law and father in law ( they have visitor visa for 10 years multiple entry)- visitor visa extension thr.online

I started yesterday , online application ,

as per their document checklist
Require
digital photo
Upfront medical
Biometric
Asking restoration fees -$200
( is it required???)
So my question is
1. Is it always required upfront medical or it depends on age
2. For restoration statues - yes or no - how this matters
3. Can I apply for both under one application

How long have they been in Canada? When does their visitor record expire? How many extensions have they received?
