VFS consent form after PPR

Hi,

Recently, my parents visitor visa got approved. They applied from India. The email says to attach VFS consent form, courier and sms form.
I got these 2 forms from the Internet.

Just confused about what pages in the consent page to be filled? Is it page 2 and 3? (Indirect submission on page 3)
