That thread is 18 months old and some of the information listed isn't necessarily correct. If you don't use a Representative, you do not send the form. Why in the world would you confuse CIC by sending in a blank form? They will think you forgot to fill it out.

Any advice from the Call Centre is suspect as best. They give bad, wrong advice all the time.
 
I asked CIC and they said don't include forms that don't apply to you. So you can write one statement with the Document Checklist and say you didn't include certain forms because they didn't apply. I don't see the harm in that or even include the forms but cross it out with a N/A in the center. What's the harm in that?

This is what the application guide recommends you do with forms that do not apply to you. This way they know that it doesn't apply and that you didn't just forget to include it.

I have searched both 3900 and 3999 with Acrobat Reader. I can find no reference to such an instruction.

In fact, it states in both 3900 and 3999: (the highlighting is CIC's)

used the services of a representative to help you prepare or submit your applic ation; or
are appointing a representative; or
are cancelling a representative's appointment.

I found the same info. Not sure why people give the wrong information.

I just put N/A on the checklist and thus, it was fine.
