I have misplaced my IMM-5292 which is needed as part of the documents need for citizenship and was in the process of filling out the Verification of Status or Replacement of an Immigration Document [IMM 5009]

However I was unsure of what to fill out on the forms and hoping someone would know the answers:

20) Immigration document date of issue (what document are they referring too?)
21) Canadian port of entry - Place of issue (I entered Canada via Vancouver, is that what I should enter or does this relate to 20?)

You need this document to apply. You may have lost the original but did you make any copies of it and still have those copies? If not, then you'll need to wait until you get a new one. I'm pretty sure they will send back your application if you don't have it.

Unfortunately I do not have a copy of the IMM-5292 form, but question (20) requires a date, would this date be the day that a person entered Canada for immigration?
 
I assuming it is COPR issue date or the date of the immigrant visa in your passport (if you have one)
