use of representative

hi everyone pls help

the cic msg me for the Use of representative form though i do not have any representative, i am the one who processed my paper .

i am thinking is it due to my third party payor? i just use her card to pay for processing and PR fee because i do not have a credit card to pay the mentioned fees thru online then paid her later on cash

pls see what it says :

We have completed an initial review of your application and now require the following information/documents in order to continue processing:  A detailed written explanation regarding your relationship with your third party payer, "owner's name", and documentary evidence to support this;  A signed and completed Use of a Representative form (IMM5476). To learn more about individuals who can represent you, visit our website at http://www.cic.gc.ca/english/information/representative/index.asp. Please note that failure to disclose use of a representative is considered misrepresentation which may lead to the refusal of your application.
