Use of a representative form? What's the use of it? Should I fill and submit it?

I have filled the information on all the forms I need and I have the Use of a representative form to go. No one helped me fill up my papers. Should I still assign the person who will sponsor me for example? What other uses does this form have other than stating who helped you fill your forms up and whether they did it for free or not?

Would really appreciate a response.

You dont need to fill it up if you dont have a lawyer/consultant, therefore you dont need to include it in the application at all. It's only an option if applicable, which is not in your case.
