I'm an American citizen applying for a temporary work permit to Canada. On the IMM 1295 form, it asks if you are an lawful permanent resident of the United States. When I check that I am, the form requires a USCIS number and an expiration date. Should I just leave this blank even though it is marked as required?
The form also asks for a national identity document number. Should I check that I don't have such a document given the US doesn't have a document called "national identity document"?
