Upfront Medical - What Documents Required ?

Dear Seniors, Friends, Colleagues

I got a job offer in Quebec and my company agent asked me to do Upfront Medical,
They have not given me any number, just asked me to go to the Approved Medical center in Delhi and tell them Upfront Medical.

I don't have an IME (IMmigration Medical number) so How does the Doctor upload my medical report on the IRCC portal.
anyone has faced similar situations..pls advise. My CAQ, LMIA, and PCC all done/Approve.

As no one replied, and I already got upfront medical results. I would let you know this process is very simple just go to any canda immigration approved clinic doctor and ask them to do upfront medical. With your passport they will do it. They don't need more documents.
