Hello everyone!

I am about to have the upfront medical next week and have a quick question...
The website shows that I need to bring:
- Any specialist and/or surgical report (in English).

Does this have to be an actual copy?
I have a hyperthyroidism and just seen a specialist recently but did not receive any record.
Do I have to specifically ask for the record to provide to the panel physician?

Thank you

Call the Dr and ask. All clinics ask for different things.

I believe you would have got a TSH check done, in case you did, carry all those medical records.

My understanding is that if you have had any surgery or hospitalization, you need to take your discharge summary. Apart from that, I am pretty sure that they won't go by the blood test reports or scans that you have done any ways and would order their own tests for a specified condition. So I believe you don't need to worry about taking anything and just disclose whatever health/chronic conditions you have honestly.
