I am wondering if I can go ahead with an upfront medical exam (will be applying with my spouse). However, I am not sure if Express Entry applicants with spouse are eligible for an upfront medical exam. On the Medical exam page, it says "A new application process for the spousal sponsorship program came into effect on December 15, 2016. Spouses, partners or dependent children can no longer do an upfront medical exam. Wait for our instructions before going for your medical exam." - However I don't know if it applies to express entry applicants (who will be applying for both themselves and spouse) or an applicant who is already in Canada, let's say Canadian citizen but is sponsoring their spouse?
