Updated Information Chart

I have received my confirmation email and in Appendix A it has the "Update Information Chart".

My question is, if none of my information has changed do I still need fill this in at all?

Hello, i got a message in my account with instructions to submit passport for visa, they have also asked for the updated info chart but they did not attach it. Where can I find it?
